What Your Physical Exam Can Tell Us About Cortisol, Stress, and Hormone Health

Lab testing can tell us a lot about your health, but before I ever order a specialty test, there are clues sitting right in front of us.

Your body composition. Your skin. Your hair and nails. Your blood pressure. Where you tend to gain or lose weight. Signs of muscle loss. Changes in pigmentation. Even how you feel when you stand up quickly.

None of these findings can diagnose a cortisol or hormone disorder by themselves. But when they're combined with your symptoms, medical history, lifestyle, medications, and appropriate laboratory testing, they can help us decide where to look next.

This is one of the reasons I believe a good physical exam still matters.

Cortisol Is About More Than "Stress"

Cortisol is a glucocorticoid hormone produced by the adrenal glands and regulated through communication between the hypothalamus, pituitary gland, and adrenal glands often referred to as the HPA axis.

Cortisol plays a role in your:

  • Stress response
  • Blood sugar regulation
  • Blood pressure
  • Immune and inflammatory responses
  • Sleep-wake cycle
  • Metabolism

Cortisol also follows a natural circadian rhythm. Under normal circumstances, levels tend to be higher around waking and gradually decline throughout the day.

Problems can arise when cortisol production or its daily rhythm is disrupted. But symptoms alone cannot tell us whether cortisol is truly high or low, which is why testing and clinical context matter.

Body Composition Can Provide Clues

Where someone carries body fat isn't enough to diagnose a hormonal imbalance, but changes in body composition can sometimes point us toward metabolic or endocrine pathways worth investigating.

More Weight Around the Midsection

Increased abdominal or visceral fat can occur alongside metabolic changes such as insulin resistance and hyperinsulinemia. Visceral adipose tissue is also metabolically active and communicates with the rest of the body through inflammatory signaling molecules called adipokines.

Persistently elevated cortisol, as occurs in conditions such as Cushing syndrome, can also produce characteristic changes in fat distribution.

For the average patient with abdominal weight gain, however, we shouldn't assume that cortisol is the cause. Sleep, nutrition, insulin sensitivity, genetics, menopause, activity levels, medications, and numerous other factors can contribute.

More Weight Around the Hips and Thighs

A "pear-shaped" or gynoid fat distribution is characterized by greater fat storage around the hips, thighs, and buttocks.

Sex hormones influence where the body tends to store fat, which is one reason body composition often changes during puberty, pregnancy, perimenopause, and menopause.

Again, body shape isn't a diagnostic test for "estrogen dominance." Instead, it becomes one small piece of a much larger hormonal and metabolic picture.

What About PCOS?

PCOS is another condition where metabolic health, reproductive hormones, sleep, stress physiology, and body composition can overlap.

Some research has identified differences in HPA-axis activity or cortisol patterns among certain women with PCOS, but there isn't one universal "PCOS cortisol pattern."

When evaluating someone with suspected PCOS, I'm much more interested in the entire clinical picture: menstrual cycles, signs of androgen excess, insulin resistance, metabolic markers, sleep, medications, nutrition, stress, and appropriate laboratory testing.

Possible Signs of Excess Cortisol

Chronically elevated cortisol can produce physical and metabolic changes, although these findings become particularly important when we're considering true hypercortisolism rather than everyday psychological stress.

Depending on the cause and severity, signs can include:

  • Increasing abdominal weight
  • Changes in body composition
  • Muscle weakness or loss of muscle mass
  • Mood changes
  • Elevated blood pressure
  • Changes in blood sugar
  • Changes in cholesterol and triglycerides
  • Bone loss
  • Skin changes

These findings deserve appropriate medical evaluation rather than automatically being attributed to "adrenal dysfunction."

Possible Signs of Low Cortisol

True cortisol deficiency can occur when the adrenal glands themselves aren't producing enough cortisol or when signaling from the pituitary or hypothalamus is impaired.

Symptoms may include fatigue, weakness, low blood pressure, dizziness or lightheadedness when standing, weight loss, gastrointestinal symptoms, and difficulty tolerating physical stress.

These symptoms overlap with many other conditions, which is why laboratory evaluation is essential when adrenal insufficiency is suspected.

Physical Exam Findings That Make Me Look Deeper

One finding I pay attention to is orthostatic blood pressure.

If someone becomes dizzy or lightheaded when they stand, measuring their blood pressure and heart rate while lying down and again after standing can provide useful information.

But orthostatic symptoms aren't specific to cortisol. Dehydration, medications, anemia, autonomic dysfunction, cardiovascular conditions, and other factors can also contribute.

Skin pigmentation can provide another clue. For example, certain forms of primary adrenal insufficiency can cause increased pigmentation because of changes in ACTH signaling.

Other findings involving the skin, hair, nails, muscle mass, edema, or mucous membranes may not point directly toward the adrenal glands but can help reveal nutritional, metabolic, endocrine, or systemic problems that deserve further investigation.

Look at Tissues With Rapid Turnover

One clinical principle I love is paying attention to tissues that either turn over quickly or are particularly vulnerable to metabolic changes.

That means looking carefully at the:

Skin and mucous membranes: Changes can sometimes reflect nutritional deficiencies, inflammation, endocrine changes, or systemic disease.

Hair and nails: Hair loss, changes in texture, brittle nails, or abnormal nail findings can provide clues about iron status, thyroid function, nutrient status, stress, and other health concerns.

Muscle and bone: Changes in muscle mass, strength, and bone health can provide important information about metabolic and hormonal health.

Nervous system and senses: Reflexes, sensation, balance, cognition, and other neurological findings can sometimes reveal problems that wouldn't necessarily appear on a standard blood panel.

The goal isn't to diagnose someone by looking at their fingernails. It's to collect clues and then determine which ones deserve further investigation.

The Bigger Question: Why Is the System Dysregulated?

This is where root-cause medicine becomes valuable.

Instead of stopping at "your cortisol is abnormal," I want to understand why the pattern exists in the first place.

Depending on the individual, areas worth investigating may include:

  • Chronic psychological or physiological stress
  • Trauma and adverse early-life experiences
  • Sleep disorders or circadian disruption
  • Insulin resistance and metabolic dysfunction
  • Hormonal transitions or endocrine disorders
  • Chronic or acute infections
  • Gastrointestinal dysfunction
  • Nutrient deficiencies
  • Medication effects
  • Environmental exposures
  • Mitochondrial dysfunction and oxidative stress
  • Genetics
  • Inadequate nutrition or excessive exercise

Not every person needs to investigate every pathway.

That's exactly the point.

Functional medicine shouldn't be about ordering every specialty test available or blaming every symptom on cortisol. It's about using your history, symptoms, physical findings, conventional testing, and—when appropriate—more specialized testing to narrow down the most likely contributors.

Your Body Is Giving Us Data

One of the biggest mistakes we can make in medicine is looking at a laboratory report without looking at the person sitting in front of us.

A physical exam doesn't replace laboratory testing, and a single physical finding rarely gives us a diagnosis.

But your body is constantly giving us information.

The distribution of body fat, blood pressure changes, skin and hair health, muscle mass, pigmentation, and other physical findings can all become pieces of a much larger puzzle.

The goal isn't to label every symptom as "adrenal fatigue."

The goal is to ask better questions, identify patterns, test appropriately, and figure out what is actually driving the dysfunction.

Because when we understand the why, we can build a much more strategic plan for what to do next.
